Shifter or derailleur problem

A few rides ago i changed the derailleur, chain and cable on a SRAM 11 speed NX and worked well. Last ride I could not get into 11th gear and it jumped several gears. Putting onto the bike stand the derailleur almost lines up with gear 11 and opening the limit screw does not help. Gears move down to 1 but the shifter only goes through 8 clicks. Do you think there is a problem with the shifter or derailleur? Derilleur looks undamaged, bike not dropped or banged and hanger looks straight.

Reset the cable tension and all seems well. Don't understand how it got out

Have any of the ferrules on the outer cable become dislodged? This will alter your cable tension. If that has happened and you leave it, then muck will get in and eventually lead to sticky shifting.
