(KSL 1) My friend had main pivot bolt unscrewed for some time (on a drive side) and it was touching chainring/chain a bit, it looks like this now:

how did that bolt look new? does it have extra flange to cover a bearing ? because it doesn't cover it.
a bit strange that it's empty inside, does it consist of 2 elements? that bolt + some covering plate ?

They are renowned for coming loose, especially as you have to take the chainring off to check the tightness.
I marked mine with paint for a visual check.
Looks like the plastic washer has worn away that prevents the chain hitting the bolt head.

so that was a plastic cap/cover on top of it? just glued to an alu bolt ?

is it glued to the bolt? how is it attached? or its lip is under the bolt ?

Clips over perhaps? Dunno, never taken it out completely.

I'm curious how/if the OP situation got resolved? I have a drive-side main pivot bolt that looks similar, and it turns out the threads in the "nut" embedded into the frame are shot. LBS says that the frame needs to be replaced. But it's out of warranty so I'm wondering about cheaper options. Anyone know if this sort of thing is repairable? I'm not a machinist, but wondering if drill, tap, and then something like a helicoil might be possible. Or maybe a machinist would be needed to create a new bolt.

it's not my bike, i wanted to help a friend of a friend, I don't know what happened later. bike still works, only that bearing is less covered i guess.

That needs a separate "post", you will attract more answers. (The proper word is "thread", but that would be confusing!)

Thanks guys, I should have thought to create a new thread in the first place. I ended up "having" to go the expensive route and give the frame back to Spesh in exchange for a discount on a new bike. The pivot bolt would not tighten up at all and it was looking like a complicated and expensive route to find a machinist to reconstruct new larger threads and fabricate a new bolt. It was not clear it would work. So since it was my only non-road bike, and I didn't want a lot of down-time, I went the new bike route. I've now marked the blasted bolts with a paint pen on the new one.
