Lots of foam/soft cloth around the LHS chain/seat pivot. Bike solidly supported horizontally at the pivot. Drift the loosened axle out with increasing force till it moves while turning the Maxle if possible. good luck with it.

The cup/conical washer is dictated by the caliper. For example if you were using a Guide caliper instead of Guide RE caliper then the cup/cone spacer would not be required.

You asked about the excessive axial play in the cassette. That should not be there. Either the cassette is coming loose or the freehub bearing is begining to collapse, or both. Easy to check. Free hubs can make some pretty strange noises when they are failing. Good luck with it.
